As you may know, in 2020, the American Heart Association released a presidential advisory highlighting a concerning trend in rural America: rural residents have a 30% higher risk of stroke and a 40% higher risk of heart disease, and they live three years fewer than their urban counterparts, on average.
As the leaders of rural hospitals, you all clearly see the unique challenges of providing equitable, consistent and evidence-based care to high-acuity, low-frequency cardiac and stroke patients. That's why the American Heart Association is working with rural hospitals like yours to blaze new trails that address these challenges.
We ask that you join our coalition of more than 400 Pathfinder Hospitals to continue advancing rural patient care together.
The Rural Health Care Outcomes Accelerator is a no-cost program connecting hospitals across the country with robust resources to learn and share insights scaled to the rural setting. Our singular goal is to support hospitals like yours in improving the overall health and outcomes of stroke and cardiovascular patients in rural communities.
No-cost participation includes:
3-year access to add new quality improvement programs: Get With The Guidelines® - Stroke, Coronary Artery Disease and/or Heart Failure
· Dedicated 1:1 rural consultative quality improvement support
· Peer-to-peer learning collaboratives and the Association's Rural Community Network
· Access to American Heart Association professional membership and scientific council
· Rural award recognition and marketing toolkits
· Continuing education through the Association's Professional Education Hub
· Adherence to the reporting requirement for disease-specific health care certification programs
Participation in the Accelerator is valued at more than $25,000 annually, and available at no-cost to eligible hospitals. Together we can make a profound difference for those we serve and chart a path toward improved outcomes for rural communities.
